Call Shifts
Weekday: 7:00 AM - 5:00 PM and 5:00 PM - 7:00 AM
Weekends: Fri 5:00 PM - Sun 7:00 AM and Sun 7:00 AM to Mon 7:00 AM
If you get an early morning consult (6:30-7:00a a.m.), gauge how long it will take you, the goal is to be less than an hour late to clinic, if you need assistance, please communicate with the inpatient consult resident
If you had a rough night, get some sleep and aim to join clinic at 10:00am, please communicate with the senior at clinic that you will be late
Staffing
Staff with the senior on call first
If determined necessary by the senior, staff with the fellow on call for the specific specialty listed
Follow Up
ED Consult (not being admitted):
verify the patient's best call back number
open "doc/orders" and place RTC orders
email "GRP-Follow up" with the patient's name, MRN, insurance, follow up interval and which provider (e.g. academic (specify) clinic or attending name if private)
Inpatient (or ED being admitted):
verify the patient's best call back number
open "doc/orders" and place RTC orders
add patient to shared "inpatient consult
Other
Avoid bringing patients over to PAV after hours unless you need additional testing (e.g. OCT, optos), patients cannot be left alone at PAV
If you notice ED Annex is missing supplies, try to restock it or let the consult resident know so they can communicate with Deborah Soto
Always leave the clinic and annex the way you found it (e.g. clean off probes, turn off machines)

Corneal Culture kit: Call Microbiology Lab: 714-456-5439, ask for an "eye kit" to be tubed to Tube station #313 in ED Annex
Fortified antibiotics for ulcers in the ED: Type "fortified" into the order tab and order Vanc and Ceftaz (most commonly)
Fortified antibiotics for ulcers in clinic: Type "fortified" into the order tab, order Vanc and Ceftaz and send it to Yorba Linda specialty pharmacy .
Natamycin for fungal ulcer: send the order to UCI pharmacy. Other pharmacies will not have this medication
Amniotic membrane graft or Prokera for SJS or burn patients: Type "Tissue request" into the orders tab-> select the necessary item and number of items--> print the order requisition--> walk this piece of paper to the 3rd floor Blood Bank--> they will give you the items (takes 15min) inside of a giant box filled with dry ice. These items will last 24 hours within the box. Email Marlen and Deborah Soto the MRN and which items you ended up using along with their serial numbers AND indicate which eye each serial number was used for.
Intravitreal or subconjunctival medications (eg for endophthalmitis): Type "intravitreal" into the order tab, order the necessary antibiotics (usually Vanc and Ceftaz)--> call the IV Room 714-456-5981 --> will take at least 30 minutes or more --> pick it up from inpatient pharmacy which is on the 1st floor of douglas hospital near ED Annex
After-hours appointment (e.g. weekend/ holiday post op or corneal check): Instruct the patient to come to douglas hospital lobby atrium at a predetermined time and tell the front desk attendant to page the on call ophthalmologist (tell them to google maps "DH cafeteria")
Follow below steps to add Michael Han (MH) "Order Set"
Inpatients
It is advised to join the fellow/ attending while inpatient for all surgeries however consults are still the priority
Under "orders" click "new" > add the appropriate MH "Order Set"
Must specify "UCI OR MAIN", "Inpatient Surgery", the TDOS and change the surgeon name and procedure as appropriate
Call Main OR Scheduling (M-F 8-5p only) for future surgery dates
Call OR Front Desk (all hours) for same day add-on surgeries or after-hours future surgery dates
Notify primary team, order NPO at MN and check meds for anti-coag
Day of surgery, mark patient and put in H&P Note
ED (Outpatient)
Open "doc/orders" > Follow the same instructions as above
Obtain Consent:
Open the procedures tab and toggle to Consents OR toggle to the "rarely used" option and click Consents
Generate a consent, make sure all fields are filled out, then sign it with the attending/ fellow and the patient. You do not all need to sign it at the same time because you can re-open this page after you sign your name. You may reopen this page under the Consents tab on Haiku and have the patient/attending sign at bedside.
